Thank you for choosing Rehab Institute of Wisconsin Services for your rehabilitation needs. We look forward to working together on your journey for a safe and healthy recovery.
To maximize every moment of your first therapy session, we have made available our new patient paperwork for you to complete prior to your appointment.
Please click on the forms below. Please complete them and bring them to your first appointment.
If you have questions about the paperwork, please call our office or you may arrive a few minutes early to your appointment.
You will need Adobe Acrobat to view and print the forms on this page. If you do not have Adobe Acrobat (PDF), you can download it for free here.
Call us to schedule your first visit, which will include evaluation and the development of a plan of care, based on the evaluation findings. We can call your insurance to verify benefits and inquire if a prescription is needed from your physician.
Wear or bring with you clothes that will be comfortable for you to move in. Shorts are helpful for treatment of the lower extremities and a short-sleeve t-shirt or tank top works well for shoulder or other upper extremities treatment. We do have hospital gowns too, if you prefer to change into one of these.
Your plan of care will be individualized to address your particular problems and goals and the deficits found during your evaluation. Physical and occupational therapy may consist of the following:
- Manual therapy techniques
- Therapeutic exercise
- Therapeutic activities
- Neuromuscular re-education
- Gait training
- Patient education
- Home exercise program instruction
For the majority of patients we treat, we typically avoid working through increased levels of pain. There are exceptions to this, however, that are dependent upon the type of musculoskeletal injury you've had and the treatment goals and indications. This will always be discussed with you and your therapist will work within your tolerance. It isn't unusual to experience temporary soreness after the sessions, just as you would after exercising.
Each session will last about one hour.
Yes, we generally offer treatment from 7 a.m. to 6 p.m. We will work with you to accommodate your scheduling needs, if they are outside of this range.
Yes our Brookfield location has plenty of free parking right outside the doors.
We will be happy to contact your insurance company, on your behalf, to verify our network status and your benefits.
